* Applicant must be at least 21 years of age.

* Complete the South Carolina Department of Social Services Consent to Release Information form.
* The 1–9 Employment Eligibility Verification form must be signed.

Also, include the following:
* 10-Year Driving Record (obtain from the Department of Motor Vehicles)
* Social Security Card (must present original document, photocopy not allowed)
* South Carolina Driver’s License (must present original document, photocopy not allowed)
* High School Diploma/GED, or sealed transcript (original must be witnessed)
