- * Complete the Authorization for Criminal Investigation (SLED Check).
- * Complete the South Carolina Department of Social Services Consent to Release Information form.
- * The 1–9 Employment Eligibility Verification form must be signed.
- * Complete the Reference List Form.
- It is very important that you have COMPLETE ADDRESSES (street with number, city, state and zip) for these references. Three (3) Personal Reference Forms completed by the references listed.

- * 10-Year Driving Record (obtain from the Department of Motor Vehicles)
- * Social Security Card (must present original document, photocopy not allowed)
- * South Carolina Driver’s License (must present original document, photocopy not allowed)

- * W-4 Form
